Global Handwashing Day Contest Release Form

This release form is for those participating in the “Creating Clean Hands” Contest.  The Global Handwashing Partnership must have the required written release form on file for your submission to be counted. This authorization assures that those featured in your music video know that the video may be shown publicly. Complete one release form for every person shown in the video using the link here. Written parental consent is required for any person under 18 years old. If authorization is not provided, your video submission will be disqualified.
